Hello doctor,

I have gallbladder mass and jaundice. Please help.

I have gone through the above reports (attachment removed to protect patient identity).

Before going further, I would like to know a few details,

1. What is the general condition of the patient?

2. Any other illness like diabetes, hypertension, and, kidney problems?

As per reports, it looks like gall bladder cancer. It is locally advanced probably unresectable because of jaundice. I would like to see films and patient myself though. The first step, in this case, to bring down bilirubin by putting PTBD (percutaneous transhepatic biliary drainage), which I think must have been advised. If the general condition is good and bilirubin comes down to normal, we can start chemotherapy and also look for the possibility of surgery after four cycles of chemotherapy. Both surgeon and medical oncologist should see a patient decide a proper line of treatment. If the disease is metastatic, then there are no roles of surgery and only chemotherapy is advised. It helps in controlling the disease for the time being. If the condition of the patient is not good, then only palliative care is advised.
