Hello doctor,

I have a severe inner thigh rash, which I believe is jock itch, and three pimples (possible warts) in different places, but all in the skin fold between my thigh and groin. I have tried everything, including Daktarin powder, Lamisil spray, Daktacort cream, Tea tree anti-fungal gel, and regular washing and trying to keep the area dry, with no success. In the majority of cases, these treatments aggravated my symptoms, causing pain and swelling. I have had this breakout for about three weeks. Any suggestions or advice would be greatly appreciated. I have previously had outbreaks of inner thigh rash. I used to play a lot of sports, and outbreaks are usually caused by excessive sweating or activity. Currently I am taking tablet Salbutamol for asthma.

The swelling in the photo could be caused by warts. Skin tags are another possibility. In either case, your dermatologist may need to intervene. Use cryotherapy or cauterization instead of an excision biopsy. If there is pain and swelling as described, jock itch or intertrigo may have caused a bacterial infection. You may require topical antifungal cream (such as Clotrimazole or Luliconazole) in addition to topical antibiotic cream (such as Mupirocin or Fucidin). If it does not respond after consulting your doctor, you may also take an oral antifungal, such as Fluconazole 150 mg weekly for four weeks. Fungal infections are known to reoccur. Other precautions that may be useful include:

1. Wear Loose cotton inner wears.

2. See that there is no retention of Moisture.

3. See that there is adequate ventilation.

4. Avoid tight fitting wears and tight innerwears.

5. Wear boxer type shorts.

6. Clean the area with warm water twice daily. keep the area dry. Avoid friction or irritation or rubbing. You can continue to use antifungal dusting powders to prevent recurrence.
