Hi doctor,

My wife is suffering from black spots on hands, back, and legs. I have attached the photos for your reference. She is suffering from this disease for three years. These are itching too. All her blood tests are normal.

I have read your query in detail and viewed the attached pictures (attachment removed to protect patient identity).

Is she suffering from any other problems like diabetes, high blood pressure?

Does she take medicine for any other disease?

Please answer these questions so that I may suggest you tablets according to that.

Probable diagnosis

These are spots of lichen planus. It is a skin disease which causes lots of itching.

Treatment plan

1. She can apply Tenovate-GN cream (combination of Clobetasol propionate and Neomycin sulfate) over these lesions. 2. She can tablet Levosiz 10 mg (Levocetirizine hydrochloride) can be taken in the morning and evening. Consult your specialist doctor, discuss with him/her and take the medicine with their consent.

I suggest her to take tablet Defcort 6 mg (Deflazacort) three times daily for one week to stop the progression of the disease.

Treatment plan

1. Along with Defcort you can take tablet Levosiz 10 mg daily for two weeks. 2. Apply Tenovate-GN cream over these lesions twice daily. 3. You may also apply Calamine lotion like Calosoft for symptomatic relief once or twice a day.

Preventive measures

Avoid scratching as it may increase these lesions.
