I am reaching out regarding my brother. He is 48 years old and was recently diagnosed with stage 4 lung cancer. What has been particularly surprising for us is that he did not experience any significant symptoms beforehand, no persistent cough, pain, or other noticeable issues. He initially underwent a scan due to some back discomfort, and the diagnosis came as an unexpected shock.
We are trying to understand how the disease could progress to this stage without clear warning signs.
Why did my brother get stage 4 lung cancer with no symptoms?
Does this suggest that the cancer may have been developing silently for a long time, or that earlier symptoms were too mild to be recognized?

Unfortunately, what you have described happens more often than most people think. Sometimes, lung cancer can grow and spread for a long time without showing any clear signs. There aren't many pain receptors in the lungs, so a growth can grow there for a long time without hurting or making it hard to breathe.
Most of the time, signs don't show up until the disease is far along or has spread to other parts of the body. Sometimes the first signs aren't connected to the lungs, like back pain, bone pain, or general tiredness. These symptoms lead to imaging tests, where the cancer is found by accident. Often, lung cancer is found during tests that were done for something completely different.
Early signs can also be very subtle and easy to miss. Mild fatigue, a cough once in a while, or shortness of breath may be caused by normal things like worry, getting older, or minor illnesses, which let the disease get worse without being noticed.
So, yes, a stage 4 diagnosis usually means that the cancer had been growing slowly for a while before it was found. This does not mean that anyone missed something or did something wrong; this is just how lung cancer can act sometimes.
