Hello doctor,
I am attaching my blood report. My lymphocyte levels are again in the same range, and the peripheral blood smear also shows relative lymphocytosis. This has been the result for four years in a row. I have no other symptoms, and I am really concerned about what may be the reason.
Could you please advise me on the possible reasons and whether I should undergo more tests?

As seen in your report (attachments removed to protect the patient’s identity), your CBC (complete blood count) shows very mild lymphocytosis, and the peripheral blood film confirms this with normal morphology. This normal morphology rules out anything serious, so there is no need to worry about it.
However, your Hb (hemoglobin) is 16.9, and your Hct (hematocrit) is 49, which are on the higher side. If you are a smoker, please stop. You can repeat your CBC after three months to check for any changes.
Isolated lymphocytosis is common. The primary goal is to use history, physical examination, peripheral smear, and flow cytometry to rule out a clonal disorder such as MBL (monoclonal B-cell lymphocytosis) or CLL (chronic lymphocytic leukemia), which is a frequent cause in asymptomatic older adults. In younger, symptomatic individuals, a reactive cause, especially viral, is most likely.
Based on this report, there is nothing serious. You can follow up with a CBC every three to four months to monitor for any significant changes. In view of the high hemoglobin, secondary polycythemia is one possibility, and smoking is a common cause. Therefore, stop smoking and repeat the test after three months.

Thank you for the reply.
I have been a non-smoker for three years now. Do you think that could still be the reason for the elevated numbers? Also, Doctor, below are the readings from the past four years. Lymphocytes have always been high. Do you think this is still normal? These are my past reports:
Three years ago: Lymphocytes 54%, absolute count 5.2 × 10³/µL.
Two years ago: Lymphocytes 52%, absolute count 4.8 × 10³/µL.
One and a half years ago: Lymphocytes 53%, absolute count 4.9 × 10³/µL.

I read your query and understood your concern.
As mentioned earlier, smoking is one cause of high Hb. Repeat the CBC after three to four months. Your lymphocyte count is only very slightly high, so please relax. Even repeating the CBC every six months will be acceptable.
