Hi doctor,

I am currently on holiday (in a rural forest area) and have a medical concern. I had a splenectomy last year, and I am worried because I have been bitten by mosquitoes a few times. I know this increases my risk for severe malaria. It has been less than a week since the bites, and I do not have any symptoms yet.

I am considering getting Doxycycline from a local pharmacy as a precaution, but I am unsure about the reliability of the pharmacies here. Should I start taking Doxycycline just in case, or is it better to wait for symptoms or try to access higher-quality medical care?

Thank you for reaching out, and I completely understand your concern. Being asplenic does place you at higher risk for severe infections, including malaria, especially in high-risk regions. Given your recent mosquito exposure, it is wise to think ahead.

You have had several mosquito bites in a malaria-endemic area while being asplenic, which increases your risk of developing severe malaria. The spleen plays a crucial role in filtering parasites and bacteria; without it, infections can progress more rapidly and severely.

At this stage (before symptoms), no tests are required. However, be prepared to seek immediate medical attention and testing if any of the following symptoms develop: fever, chills, fatigue, or weakness.

Tests that may be needed include:

  1. Rapid malaria antigen test.

  2. Peripheral blood smear for malaria parasites.

  3. Complete blood count (CBC).

  4. Liver and renal function tests (if treatment is started).

I suggest the following treatment options for you.

  1. If you are not already on malaria prophylaxis, consider starting it as soon as possible.

  2. Doxycycline 100 mg once daily is commonly used for malaria prevention and may be appropriate if obtained from a reputable pharmacy.

  3. Try to wait until you reach a town with a trusted clinic or hospital to obtain medications, as quality can vary in rural areas.

  4. Avoid starting any medication without proper medical supervision, especially from questionable sources.

  5. Continue your Penicillin prophylaxis as prescribed to protect against encapsulated bacterial infections.

  6. If you develop a fever or any other symptoms, seek immediate medical testing and treatment; do not delay.

To prevent further exposures, please follow these measures:

  1. Use mosquito repellents containing DEET (N,N-diethyl-meta-toluamide), especially in the evening and early morning.
