What do irregular periods & spotting indicate?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello, Doctor,

I am a 24-year-old woman who has never engaged in sexual activity. Typically, my menstrual cycles are irregular, but this cycle has been particularly concerning. I am currently on day 44 without menstruation, and I have experienced five separate instances of light spotting, spaced out over time. In the past, I have noticed occasional mid-cycle spotting between days 11 to 14, which my gynecologist attributed to ovulation and deemed normal. However, this time feels different due to the multiple episodes.

Additionally, I have had recurrent episodes of vulvovaginitis caused by Candida albicans. I am seeking insights into what might be causing these symptoms. Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and can understand your concern.

I have reviewed your concerns and understand your situation. Experiencing irregular spotting can be attributed to various factors, including hormonal imbalances, local lesions in the vagina or cervix such as polyps, infections (fungal or bacterial), or uterine fibroids (noncancerous growths of the uterus).

Given your history of recurrent vulvovaginal candidiasis caused by Candida albicans, it is advisable to seek treatment for any present fungal infections. Antifungal medications, either oral or topical, are effective in treating such infections.

To accurately diagnose the cause of your symptoms, I recommend undergoing the following evaluations:

  • Thyroid function tests: Thyroid disorders can significantly impact menstrual cycles, leading to irregularities.

  • Prolactin level assessment: Elevated prolactin levels can disrupt ovulation and menstrual regularity.

  • Pelvic ultrasound: This imaging can help identify structural abnormalities, such as fibroids or polyps, that might be causing the spotting.

If these evaluations return normal results, a thorough examination by an obstetrician-gynecologist (OBGYN) is essential to explore other potential causes and determine the appropriate course of action.

It is important to address these symptoms promptly to ensure your reproductive health is maintained.

I hope this helps.
