Hi doctor,

I am worried about my husband's type-2 diabetes. Today evening before dinneŕ the sugar was 317. He is taking Januvia along with Diamicon XR 60 and in night Razel along with Dimicron XR 60. The medicine is also not working very well on him. He would not able to visit a doctor for another one week due to a tight schedule. Please suggest is it right to wait for one more week. Can you suggest some medicine which can lower down his sugar?

I just read your query about your husband and from what you have mentioned it appears that your husband has uncontrolled blood sugar.

I suggest that you add the tablet Metformin 1000 mg twice a day in addition to all the drugs that you have mentioned. This is only if you know that his kidney function test was normal previously. Consult your specialist doctor, discuss with him or her and start taking the medicines with their consent.

1. He must drink eight to 10 glasses of water daily.

2. Avoid rice and heavy food like meat.

3. Take only boiled food for a few days, preferably only vegetables (not potatoes).

4. Take dry flat bread with plain low-fat yogurt sometimes as a meal.

5. Make sure the food is light.

Do his HbA1c test, and then we can review again.

I read your husband’s detailed reports (attachment removed to protect patient identity).

It appears that his HbA1c is very high, necessitating the need to add insulin to his previous medications. I suggest him to start with ten units of Insulin glargine flex pen subcutaneously once daily at bedtime. You have to learn the technique to use it from a local nurse. Consult a specialist doctor, discuss with him or her and start taking the medicines with their consent.

Keep me updated. I would like to know more about him in due course of time as to whether his sugar is controlled or not and the need for possible dose titration. I hope you know what to do if he has hypoglycemia, just in case. You must give him something to eat and get back to me for any dose changes.

Secondly, his lipid is high so that a strict low-fat diet would be best for him.

Thirdly he has low calcium. So, I would suggest him to take tablet Calcium 500 mg once daily and get daily sun exposure for 30 minutes on his bare hands and feet all for three months.

Repeat HbA1c after three months of starting Insulin.
