How to manage a delayed period?

Hello doctor,

My period is not coming. It is almost 42 days. I got my last period on before last month. But this month I got no periods. For two days, I have had pain in my back and leg, and lower abdomen. So, do these symptoms indicate that periods will come?

I read your query and can understand your concern.

Let me explain to you the International Federation of Gynecology and Obstetrics (FIGO) of the menstrual cycle.

Frequency :

  1. We call it normal when the cycle occurs between 24-38 days.

  2. We call it frequent when the cycle occurs less than 24 days.

  3. When the cycle is more than 38 days, it is called infrequent.

  4. Absence of menses is called amenorrhea.

Duration :

  1. Normal duration is up to eight days.

  2. Prolonged duration is more than eight days.

Regularity :

  1. A regular cycle is when the longest and shortest cycles are less than nine days.

  2. Irregular cycle is when the longest and shortest cycle are more than 8 days.

Flow:

  1. Normal.

  2. Heavy.

  3. Light

I suggest that you do the following:

  1. Serum free thyroxine (FT4).

  2. Thyroid-stimulating hormone (TSH) test.

  3. Serum prolactin.

  4. Serum follicle-stimulating hormone (FSH) and luteinizing hormone (LH) on the second day of the cycle

I will advise you to take the following medication:

  1. Tablet Dydrogesterone (Pregnadien derivative progestogens) 10 mg two times a day for 21 days and then a seven-day pill-free interval, start again and continue for 21 days and then a seven-day pill-free interval for three months.

  2. Tablet Folic acid (Vitamin B family) 5 mg once at night for two months.
