Hello doctor,
My wife has missed her period and is now having spotting after intercourse. I do not know why this is happening, and I am worried. Previously, she had an ectopic pregnancy about a year ago, and now she has spotting.

As per your description, these symptoms could be due to a hormonal imbalance, but they could also be due to pregnancy, although the chances are very low.
As you missed your period, it could be an anovulatory cycle (which happens when an egg does not release from your ovary) due to hormonal changes or due to pregnancy if precum contains sperm.
You should go for a home urine pregnancy test after seven days of your missed period date or get a blood beta-hCG (human chorionic gonadotropin) test done right now to rule out pregnancy.
If pregnancy is not there, it could be due to a hormonal imbalance, and you can wait and watch for your menses or take progesterone for withdrawal bleeding.
Get your thyroid profile and hemoglobin test done, also to check for any hormonal changes.
