I’m exhausted by mood swings and emptiness. What can I do?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am having a rough patch again. My mood swings are intense; some days, I cannot get out of bed, and other times I feel restless, like my brain is racing. It is completely exhausting. I am on mood stabilizers, but lately they do not seem as effective as before.

My sleep is all over the place, I barely eat, and I can not focus at work. I am not sure if this is a medication issue or just part of my mood cycle. Also, I feel emotionally numb rather than sad, and I am wondering if therapy could help with the depressive side. I hate how unpredictable this gets.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

I am really sorry you are going through this. It sounds exhausting and overwhelming. When your mood swings between feeling unable to get out of bed and feeling restless or mentally on fire, it often means your current mood stabilizer may no longer be fully controlling your cycle. Medications can lose effectiveness over time or may need adjustment, especially if your sleep, appetite, or focus has changed. This is not your fault; it just means your brain chemistry needs a little extra support right now.

Feeling empty rather than sad is actually a common part of depressive phases, and therapy can help more than many people expect. Therapists can guide you to process your thoughts, rebuild routines, and reconnect with things that once felt meaningful.

I know the unpredictability makes everything feel harder, but with the right medication review and ongoing psychological support, many people regain stability. It is important to speak with your psychiatrist about these recent changes in sleep, eating, work focus, and mood so they can adjust your plan if needed. You deserve relief, and there are real, effective options to help you feel steadier and more in control.

I hope this answers your query.

Please let me know if I can assist you further.

Thank you.
