Hello doctor,

I am a 62-year-old male diagnosed with rheumatoid arthritis five years ago. Recently, I noticed increasing pain and stiffness in my fingers, wrists, and shoulders, especially in the mornings, lasting for more than two hours. My current regimen includes Methotrexate (25 mg weekly), Hydroxychloroquine (200 mg daily), and occasional Ibuprofen for flare-ups, but the symptoms have been worsening over the past six months. My recent blood tests showed elevated ESR (erythrocyte sedimentation rate) 65 and CRP (C- reactive protein) 14, with persistent positivity for rheumatoid factor and anti-CCP antibodies. X-rays of my hands revealed progressive joint erosion compared to last year’s imaging. I am worried that my current treatment is not sufficient to slow disease progression. Should we explore adding biologic agents or consider alternative strategies to manage the inflammation and prevent further damage?
