I have a question about sinuitis. I have been having yellow and green mucus every day. I have a history of this after sphenoid sinus surgery. I am worried about what is causing this. Kindly suggest.

Other than the mucus do you have any symptoms? Have you been having yellow or green mucus continuously after the surgery even when you were on antibiotics? Does the mucus pass behind the nose and drip into the throat also? Do you have any fever or pain? Did your ENT surgeon do a nasal endoscopy post-surgery? Did you undergo endoscopic nasal suction clearance a few times after the surgery or was that not needed at all? Do you have preoperative CT (computed tomography) scan pictures? Did they suspect fungus during the surgery? Did they give any samples for histopathological examination (biopsy) during the surgery? If yes, do you have its report? Do you suffer from allergies? Is any allergy test done to check what you are allergic to? Kindly provide us with all details so that we can help you further. Thank you.

Hello doctor,

First of all, thank you for your kind response. I appreciate your follow-up questions. Other than the mucus do you have any symptoms? A: The mucus is yellow and green. Sometimes the mucus is hard as if it contains a scab. I seem to have lost partial hearing in my right ear. I believe it is caused by the pressure. Have you been having yellow or green mucus continuously after the surgery even when you were on antibiotics? Does the mucus pass behind the nose and drip into the throat also? A: My surgery was about a year ago. I have had this continuously. After taking an antibiotic, it got better but then returned after swimming in the ocean. Do you have any fever or pain? A: No pain. I have had a fever over the past few days but I believe this is unrelated. The one thing I have noticed recently is that my right ear feels slightly blocked and my hearing ability is affected. Did your ENT do a nasal endoscopy post-surgery? A: Yes, I believe this was done. Do you have preoperative CT scan pictures? A: I do but I do not have them handy. Did they suspect fungus during the surgery? A: Yes, there was a fungus ball and polyps during surgery. Both were removed. Did they give any samples for histopathological examination (biopsy) during the surgery? Do you have its report? A: I do not have this. Do you suffer from allergies? I looked at the picture you sent (attachment removed to protect the patient's identity). You would need a course of antibiotics, nasal douching with saline plus bicarbonate, antiallergics, and steroid nasal sprays. I am writing the same below. 1. Take a tablet of Clarithromycin 500 mg twice daily after meals for 10 days. 2. Take a tablet of Fexofenadine hydrochloride 120 mg once daily in the evening for 10 days. 3. Fluticasone furoate nasal spray two puffs in each nostril twice daily for 10 days. 4. Saline nasal douching (add a pinch of baking soda to the solution each time) douche multiple times daily for 10 days. 5. Take steam if you tolerate it, can be taken twice or thrice daily. 6. Avoid cold drinks, smoke, and cold foods. 7. Take plenty of warm fluids. Review with me after 10 days. You will need a skin prick allergy testing (after seven days of abstaining from antiallergics) or a blood comprehensive allergy profile. The allergy test will tell us clearly what's causing this persistent mucus issue. I hope this helps. Thank you.
