Hi doctor,

I am a 45-year-old female. I have been battling multiple sclerosis for years, and now that I am in the advanced stage. I am struggling with severe complications that feel overwhelming. My muscle weakness has progressed to the point where I can barely move, and I am experiencing difficulty swallowing and speaking. Breathing has also become more labored, and I am worried about the risk of respiratory failure. On top of that, I have had recurring infections, including pneumonia, which are becoming harder to recover from. The pain and spasms are constant despite being on medication, and my fatigue is so extreme that it is hard to stay awake for most of the day. Emotionally, I am feeling scared and defeated, and I wonder if there is more that can be done to improve my comfort and manage these complications. Are there additional treatments, medications, or therapies that could help ease my symptoms? Should we consider palliative care or other supportive measures? I want to understand how to navigate this stage of my illness and ensure my quality of life.
