Q. What are the complications of laparoscopic myomectomy, and how long to wait before conceiving again?

Hello doctor,

I had a laparoscopic myomectomy this month to remove a one-centimeter submucosal myoma. The other two myomas are intramural. I did not face any complications three days after the procedure. Can I face complications later? Also, since my cavity was opened, how long do I need to wait before conceiving?

After the uterine cavity is opened during a myomectomy, the chances of a uterine rupture occurring during a subsequent pregnancy are very high. This can be decreased by ensuring that the subsequent pregnancy is delayed at least 6 to 12 months after the surgery. It would be best if you also had a follow-up ultrasound before trying for pregnancy.

Can new fibroids return if I wait for 6 to 12 months? I am already 38 years old and wish to start a donor cycle as soon as possible. Also, the chance of uterine cavity rupture is high during which month of pregnancy?

Six months would be the minimum waiting time to carry a pregnancy successfully. The uterus needs to heal completely else it could rupture any time during the pregnancy, the highest risk being during the third trimester.

Is the first period after myomectomy very heavy and painful? Three fibroids of 1 to 2 cm were removed in my laparoscopic myomectomy. Also, is it normal that I still experience mild spotting and sometimes cramps too? Can I get my period early?

The first period after a myomectomy can be heavy and painful but what you are experiencing now is not the period. It is bleeding following the surgery as the cavity was opened. I suggest you can start a tablet Pause-MF (Tranexamic acid and Mefenamic acid). However, please consult a physician, discuss with them and start taking the medications with their consent. You might also need a physical check-up.
