I am a 36-year-old female recently diagnosed with nonalcoholic steatohepatitis (NASH) and insulin resistance. My body mass index (BMI) is 42, and I also have polycystic ovary syndrome (PCOS). I have tried various approaches to weight loss, including intermittent fasting and glucagon-like peptide-1 (GLP-1) receptor agonist medications, but have seen minimal results. My endocrinologist has now recommended considering metabolic (bariatric) surgery, specifically Roux-en-Y gastric bypass, as a potential strategy to reverse insulin resistance and reduce the risk of long-term liver damage. However, I have a history of iron deficiency anemia and am concerned about the potential for long-term nutrient malabsorption. Would sleeve gastrectomy be a safer option from a nutritional standpoint, particularly regarding iron absorption? Additionally, how soon after metabolic surgery do patients typically see changes in hormone levels or improvements in liver function and metabolic parameters?
