Hi doctor,

Can an NCS test conducted within 15 days of possible damage to the nerve be too early to detect the damage? My son, who is eight years old, hit his right elbow last year-end and got a plaster cast on. Plaster removed after one month. On removal, his entire arm was swollen with pressure bruises, indicating plaster was too tight. He has been feeling intense pain in his elbow and wrist ever since. NCS test done after that which came out normal. But to date, the pain has not reduced. He has been taking tablet Gabapentin 100 mg ever since. Currently, he is taking syrup Uprise D3 once a week, syrup Vyzest 12 ml daily, and Calcirol D sachet 12 ml daily. MRI was done two days back showed a fracture in the distal ulna but no soft tissue or muscle damage. Nothing is mentioned in the MRI report about nerves. A tremor started in his hands after the orthopedic examined hand last month by pressing specific points. It stopped after six hours. But since the day after, hand trembles on turning hand, or on holding it sideways like when washing hands, with crepitus in the forearm. Also, after mild exertions like writing even ten words hand starts to pain and shake. How to be sure if there is any damage to long or short nerves? Please help.

I understand your concern.

I read your question carefully and uploaded reports (attachment removed to protect the patient’s identity). I would explain that there is no evidence of nerve damage in the EMG (electromyography). But sometimes, even though NCS (nerve conduction study) is normal, some small nerve fibres may be damaged, considering the pain. For this reason, besides tablet Gabapentin and physiotherapy, I would suggest taking vitamin-B complex supplements coupled with tablet Alpha-lipoic acid 600 mg daily and Vitamin E supplements to help the nerves regenerate, maybe from the compartment entrapment due to swelling. It may take a couple of months to regenerate. As the NCS is normal, there is no need for surgery as the big nerve fibres are fine.

Thanks for your reply.

Can you please suggest some medicine for a nine-year-old containing vitamin B-complex supplements coupled with tablet Alpha-lipoic acid? We, as laypersons, would not know the right one. Please guide. Also, will it heal? Are there any chances of permanent damage? Please help.

I think he will recover completely with a bit of patience and physiotherapy. I suggest you take Healthvit alpha lipoic acid 300 mg two tablets a day and Superior source children's B-Complex Vitamins two tablets per day.

Hello doctor,

My son's tremor seems to have reduced somewhat, but the pain is not decreasing enough; he still has pain while writing, brushing his teeth, eating, pushing a ball, moving his arm while dancing, etc. His regular offline school has started, and he feels pain while writing in class and missing classwork. When he feels severe pain, tremor starts but reduces on resting. MRI revealed a Type III Salter-Harris fracture in his distal ulnar epiphysis, but the orthopedic said it was nothing to worry about. But he has loud crepitus when he turns his wrist, which was not so loud before. Last time I forgot to ask about vitamin E. Can you please prescribe which vitamin E and what dose he should take along with Healthcvit alpha-lipoic acid 300 mg 2 tablets a day and Superior Source Children's B-Complex vitamins 2 tablets per day? Also, can you suggest what else we can do to help him resume his everyday life? We have stopped Gabapentin as the doctor had prescribed it for one month, and that duration is over.

I recommend continuing Gabapentin tablets to help him live a normal life without pain. Considering his current situation, I would recommend performing physiotherapy to help improve his situation. Regarding vitamin E, I would recommend taking Vitamin E 400 mg, capsule one per day. Calcium and Vitamin D can also help with the fracture healing. I would not recommend performing further tests as, in my opinion, it is a matter of time to improve.
