How to manage vomiting due to norovirus in my daughter at 8?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

Our entire family got violently ill after a cruise last weekend. Started with my eight-year-old daughter, then hit me, my husband, and my mother-in-law, within hours. The vomiting is relentless, can not keep even water down, plus terrible diarrhea that comes without warning. We are all severely dehydrated; my daughter's lips are cracked and dry. She has had 12 episodes of vomiting in 24 hours and seems lethargic. The cruise line sent an email mentioning a possible norovirus outbreak on board. My husband has a compromised immune system from cancer treatment. Please tell me,

  1. How do we manage this at home?

  2. Should I take my daughter to the hospital?

  3. How long will this last?

  4. How do we prevent spreading it further?

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

Based on the history you provided, it appears to be a severe norovirus infection, which spreads quickly and poses significant risks, particularly for children, the elderly, and individuals with compromised immune systems, such as your husband. Your daughter requires immediate medical attention.

  • Experiencing 12 episodes of vomiting within 24 hours, along with lethargy and cracked lips, indicates severe dehydration, necessitating her admission to a hospital for urgent medical care.

  • Please take her to the hospital immediately. Children can deteriorate rapidly when dehydrated, and she may require intravenous fluids.

  • Your husband will also need further evaluation at the hospital due to his immunosuppression.

  • For everyone else in the household, here are steps you can take to manage symptoms and prevent complications:

  • Hydration is essential: Even if vomiting persists, encourage her to sip small amounts (one to two teaspoons) of oral rehydration solution (ORS) every five to ten minutes. If ORS is unavailable, you can create a homemade version: mix half a teaspoon of salt with six teaspoons of sugar in 1 liter (approximately 4 cups) of clean water. Avoid sugary beverages or plain water alone, as they will not replenish lost electrolytes.

  • Watch for warning signs of severe dehydration: Signs to monitor include

  1. Dry mouth.

  2. Absence of tears while crying.

  3. Dark or absent urine for over eight hours.

  4. Rapid heartbeat.

  5. Sunken eyes.

  6. Confusion or lethargy.

Symptoms of norovirus typically last between one and three days, but the experience can feel more severe due to the sudden and intense onset. The timeline of the infection will be as follows:

Incubation period of 12–48 hours following exposure.

  • Acute illness: Generally lasts 24–72 hours (one to three days).

  • Contagious period: The virus can still be transmitted for up to two weeks, even after symptoms have resolved, though the highest contagiousness occurs while symptomatic and shortly thereafter.

To prevent the transmission of norovirus, which is highly contagious, adhere to these stringent hygiene and cleaning protocols:

  • Isolate the Infected.

  • Keep those who are infected away from others, particularly your husband, who is immunocompromised.

  • Rigorous hand hygiene, which includes washing hands thoroughly and frequently with soap and water, especially after using the restroom, changing diapers, and before eating or preparing food.

I hope this information helps you.

Feel free to ask further queries.

Thank you.
