Patient's Query
Hello, Doctor,
My 40 year old female relative, mother of 5 kids, has been experiencing headaches Numbness and tingling in the feet for the last 1 month. She also reports an intermittent sensation of coolness in her limbs and pain in her legs at rest, which is relieved by walking. She also suffers from irregular menstrual cycles in addition to these symptoms.
Her investigations, including complete blood count, erythrocyte sedimentation rate, renal function tests, vitamin B12 levels, vitamin D3 levels, and thyroid function tests, are all within normal limits. MRI of the brain is also normal. Her HbA1c is in the prediabetic range. She has hypertension. Her past drug history is significant for the use of 3-monthly contraceptive injections. Please tell us,
Kindly help.
Hello,
Welcome to icliniq.com.
I read your query and can understand your concern.
Your relative’s symptom pattern is most consistent with restless legs syndrome (RLS) with possible early peripheral neuropathy related to prediabetes. The presence of the following symptoms strongly points toward restless legs syndrome:
Tingling and numbness in the feet.
Leg discomfort at rest.
Symptoms improve when walking.
Sensation of cool limbs.
Often associated with sleep disturbance or evening symptoms.
RLS frequently occurs even when tests like
Complete blood count (CBC).
Vitamin B12.
Thyroid tests.
Magnetic resonance imaging (MRI) of the brain is normal, which matches this case.
Several factors may increase the risk of this issue in your relative’s cases,
Prediabetes (elevated HbA1c) can cause early nerve irritation.
Hypertension.
Hormonal changes or irregular cycles are possibly related to Depot Medroxyprogesterone Acetate injections (3-monthly contraceptive).
Borderline vitamin D level.
I am advising the following for your relative
Check serum ferritin level (important in RLS).
Manage prediabetes with diet, weight control, and regular exercise.
Optimize vitamin D (target >30 ng/mL).
Evaluate sleep quality and timing of symptoms (RLS is worse in the evening).
She can also take medicines like Gabapentin (an anticonvulsant medication) and Pramipexole (a dopamine agonist).
I hope this information helps you.
Feel free to ask further queries.
Thank you.
