Hi doctor,

I am 29 years old, currently weigh 285 pounds, and I can not seem to lose any weight despite trying every diet. I am struggling with my weight and need serious help. I gained 60 pounds after having my twins three years ago, and the weight has kept piling on. My knees hurt constantly, I can not keep up with my kids, and my husband is starting to make comments. I also developed obstructive sleep apnea and now use a continuous positive airway pressure (CPAP) machine, which is embarrassing. I have tried weight watchers, the ketogenic diet, and intermittent fasting, but nothing works long-term.

My primary care doctor just says, “eat less, move more,” but it is not that simple. I have polycystic ovary syndrome (PCOS), which makes weight loss harder, along with insulin resistance and prediabetes. I am considering sleeve gastrectomy surgery, but insurance requires six months of supervised weight loss first. My family has a history of diabetes mellitus and heart disease, and I am scared. I can not afford a personal trainer or a registered dietitian. My work schedule makes meal prep difficult as a single mom. Are there any weight loss medications that work?
