Is orthodontic facial profile restoration possible after extraction?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have had braces for two years and got an extraction six months ago to correct a slight flaring, but I regret it. I have a sunken face and lazy eyes now. My concerns are -

  1. Will reopening my extraction space cause relapse?
  2. Would it bring my face back to a pre-extraction profile?

Kindly suggest.

Answered by Dr. Naveen Thomas

Hello,

Welcome to icliniq.com.

Although I cannot say much without actually seeing and examining your case, you cannot go back to your previous state before extraction. It would be too much trauma to the moving teeth and may cause irreparable harm.

You can ask your orthodontist to finish your case as aesthetically as possible, then go for some smile design with veneers on the front teeth if needed.

Kindly attach pictures of your front teeth and face to give further treatment plans.

Thank you

Patient's Query

Hello doctor,

I will include pictures of my teeth. What should I ask my orthodontist to make it better?

I do not like that my face is not full and looks longer.

Kindly help.

Answered by Dr. Naveen Thomas

Hello,

Welcome back to icliniq.com.

I have seen the attached picture (attachment removed to protect the patient's identity). You can continue the treatment and close the space by pulling the front teeth to the back, and you may get an adequate smile.

The other option is if you do not want the front teeth to return to the extracted space, then ask the orthodontist to bring the posterior teeth to the space.

Still, I feel the best option is to continue with how your orthodontist is currently doing to straighten the teeth and reduce the slight tilt of your front teeth. Finally, you can ask the doctor to smooth the edges of all six front teeth to get a straight line and to reduce the height of the upper front teeth.

Thank you.

Patient's Query

Hello doctor,

My orthodontist said it is up to me, so I do not know what to choose. I liked my smile before and want it to be like that, so what should I say?

My lips and everything looked right. I am adding a picture of it for you to take a look at.

Kindly suggest.

Answered by Dr. Naveen Thomas

Hello,

Welcome back to icliniq.com.

I suggest the better option would be to pull the front teeth inside. As in the picture (attachment removed to protect the patient's identity), your front teeth seem slightly protruded and tilted.

Making them straight and aligned will give a better look, so continue with the treatment, and it will be fine. In all of the pictures of your face, there is some protrusion, so your face will look more aesthetic after treatment.

Your apprehension will go away with time as the smile will only get better.

Thank you.

Patient's Query

Hello doctor,

I have a few questions -

  1. Would pulling back the front teeth make a face even flatter and the lips smaller?
  2. Is it impossible to bring my face back to a pre-extraction profile with dental implants?

Kindly help.

Answered by Dr. Naveen Thomas

Hello,

Welcome back to icliniq.com.

If you have decided to close the space without taking the front teeth back, you can ask your dentist to bring the posterior teeth forward one by one. No need to get implants.

Your orthodontist has to bring the teeth forward one by one and close the space. Give clear instructions that you want to bring the posterior teeth forward one by one to prevent retraction, and you will get nearly the same position that you have currently.

Thank you.

Patient's Query

Hello doctor,

The thing is that I do not like my teeth alignment right now.

Kindly help.

Answered by Dr. Naveen Thomas

Hello,

Welcome back to icliniq.com.

Sorry, but it is not possible to go exactly like before. Is your doctor an orthodontist with experience or a general dentist?

Thank you.

Patient's Query

Hello doctor,

My doctor is an orthodontist and did not recommend extraction, but I still did it because I thought it was bad.

Another orthodontist told me to ask my orthodontist to torque the anterior teeth and push back teeth to the front for the top teeth and for the bottom teeth to retract to close the space because my chin is more prominent and my face looks less harmonious.

Should I go with that?

Kindly suggest.

Answered by Dr. Naveen Thomas

Hello,

Welcome back to icliniq.com.

Yes, it may be possible, bringing your aesthetics as before. It may be fine if the orthodontist has seen your case directly. But I confirm that closing the upper space by bringing back teeth to the front and lower space by retracting the front teeth may result in a deep bite.

Thank you.
