Hello doctor,

I need help with my left ear. I have been diagnosed with otitis media. I have used an acetic acid solution, ointment Cortisporin, and Neomycin drops. However, none of them have worked. Also, my ear had a white coating when examined. Kindly help.

I understand your concern. I have done through your attached pictures (attachment removed to protect the patient's identity). The pictures you have attached show a discharge in the canal. You need systemic antibiotics in addition to topical ones. If the discharge is not settling, you might need irrigation with 2 % acetic acid w/v (weight by volume) and suction under a microscope followed by administration of Clotrimazole ear drops. Also, you must prevent water from entering the ear. I hope this helps. Thank you.

Thank you for the response. Should I start an antibiotic like Amoxicillin? Also, should I continue my previous course of medicines?

I suggest you do irrigation with glacial acetic acid 2 % w/v solution, or you may instill 2 % acetic acid drops. Also, you can simultaneously use Neomycin and Polymyxin drops and systemic antibiotics like tablet Amoxiclav. I hope this helps. Thank you.
