Hello doctor,

My second baby will be two years old. Since October, she has been having yellow and green fluid discharge from her left ear. Doctors prescribed antibiotics, but the discharge did not stop. We consulted an ear, nose, and throat (ENT) specialist who cleaned her ear and advised against antibiotics, but the fluid started discharging again just two days later.

At home, we used a nebulizer and stopped giving her cow’s milk after two days the fluid discharge stopped soon after. However, the discharge started again in February, this time with a foul smell. Before the discharge starts, she usually has a fever a day earlier. Right now, she has had a cold and runny nose for the past 15 days. She does not seem to be in pain, but she is irritable and has a reduced appetite.

Regarding your child’s symptoms, I believe your child has developed persistent otitis media (OM) with possible spread to the surrounding structures, like the nose, and it may affect the throat and eyes if it is not properly treated.

Please clean the ear with hydrogen peroxide (H₂O₂) and a small amount of methylated spirit twice daily for five days. In addition, administer:

Syrup Augmentin five milliliters (mL) by mouth (per oral/PO) twice daily (BID) for one week
