Hello doctor,

I have been having lots of yellow seeds in my stool. I have had many tests for parasites which came out negative always. My only other gastrointestinal symptom is pain in the upper stomach area which I am told could be gastritis or ulcer. I am taking Prilosec for two weeks and no spicy foods. I had an ultrasound which showed no gallstones but a "liver nodule." I am having an MRI (magnetic resonance imaging) in a few weeks. Otherwise, I am healthy. My only medication is Levothyroxine 0.25 mg a day. If my pain gets bad I take Bentyl 40 mg.

I read your query and understand your concern.

As per your query, I would suggest certain medications, take them after consulting your specialist doctor. You should switch over to Nexium (Esomeprazole). Take 40 mg twice daily. However, if not relieved the doses can be increased to 80 mg twice a day. Though Esomeprazole is of the same class as Omeprazole yet trials show it to be superior to other proton pump inhibitors in treating reflux disorder. Other than that ask your gastroenterologist to add a Prokinetic drug like Levosulpiride. It is a sulpiride isomer and exerts its prokinetic effect by blocking D2 receptors. Recent studies have shown that combining it with Rabeprazole has shown a good effect in patients suffering from GERD (gastroesophageal reflux disease). Domperidone is very effective in treating GERD when combined with Rabeprazole. Acotiamide - Trials showed that 30 mg doses taken for three months thrice daily were highly effective in the treatment of functional dyspepsia. The positive effect of Acotiamide on gastric emptying, and gastric accommodation, were significantly enhanced in people taking it. Baclofen - Normally used for cerebral palsy patients and patients with other neuromuscular diseases. It has been successfully used in patients with reflux. It may decrease the frequency of relaxations of the lower esophageal sphincter and therefore decreases gastroesophageal reflux. Please discuss all these options with your gastroenterologist.

Test advice:

  1. Endoscopy to examine the inner part of the esophagus.
  2. The ambulatory acid test for monitoring the amount of acid within the esophagus.

Apart from that drastic lifestyle modification and dietary changes are required:

  1. Firstly stop taking soda or any aerated beverage.
  2. Food and beverages like chocolates, fatty food, coffee, alcohol, and peppermint should be avoided to reduce the weakening of the lower esophageal sphincter.
  3. Several food items irritate the damaged oesophageal lining and should also be avoided. They include citrus fruits, tomatoes, and pepper.
  4. Smoking cigarettes results in the weakening of the lower oesophageal sphincter and should be stopped to reduce GERD.
  5. Take about 30 grams of fiber daily in your diet. Eat one cup of fresh fruits (except banana) and half bowl of green salad daily; this will meet your extra total fiber requirement.
  6. Avoid citrus fruits as they can increase the symptoms.
  7. Avoid alcohol.
  8. Exercise daily for 20 minutes at least.
  9. Avoid oily and spicy food.
  10. Drink cold milk.
  11. Yogurt is a very good natural source of probiotics take it daily.
  12. Adhering to both medicines and strict lifestyle changes will help you in the long term.
  13. Last but not least, keep yourself fully motivated.
