Hi doctor,

I am 31 years old. I am on Metoprolol tartrate 50 mg twice daily for high blood pressure. I have a history of palpitations. I have noticed my resting heart rate dipping into the low 40s during the day, accompanied by lightheadedness and an uneasy feeling. I have confirmed it with my Garmin watch and by checking my pulse. These episodes usually last a few minutes and then will return to my usual 60 to 70 range. Is this cause for concern?

You are having symptomatic bradycardia, which is due to Metoprolol. The dose of Metoprolol should be reduced to 50 mg once a day, and it should be a sustained-release formulation. Another antihypertensive should be added instead of tablet Azilsartan or Telmisartan 40 mg once a day. It would help to have a healthy lifestyle, like avoiding fatty, oily, and high-calorie diets. Have a low salt diet and monitor blood pressure regularly, and blood pressure should not be persistently more than 140/90 mm Hg. Regular exercises like brisk walking and jogging according to your capacity at least 30 minutes a day and five days a week. Lots of green leafy vegetables, fruits, and fish once or twice a week. Avoid meat, smoking, and alcohol. Get your lipid profile and sugars tested once.
