What could cause a decrease in RBC, WBC and platelet count?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have a full blood count result with a few questions to ask about the result, but I am not sure how to attach the result so that you can see the hematological condition of the patient and help me go ahead.

The patient however, has MCV=90.9 fL, MCH=27.9 pg, MCHC=306.7 g/L, platelets count=90 x 109/L, white cells=3.2 x 109/L and red cells=3.3 x 1012/L.

I have a few questions to ask:

  1. I want to ask what the data (for red cells, white cells, and platelets) tells about the hematological condition of this patient.

  2. Does this allow me to make an initial diagnosis for this patient?

Please help.

Hello,

Welcome to icliniq.com.

I understand your concern.

I have seen the attached report (attachment removed to protect the patient's identity). His hemoglobin level, total WBC (white blood count), and platelet counts are low and below the normal range. So the diagnosis is pancytopenia.

The underlying cause for pancytopenia should be searched for. His clinical correlation is needed. If any clinical complaint is present, then mention it.

Vitamin B12 deficiency, MDS (myelodysplastic syndromes), and chronic infection-suppressing bone marrow might be the reason.

I suggest you investigate with a peripheral smear examination for further workup. If no underlying cause is detected, then a bone marrow aspiration-like higher investigation is needed.

You can consult a nearby hematologist for examination and discuss your case.

I hope this has helped you.

Please feel free to reach me again in case of further queries.

Thank you.
