Hi doctor,

Recently I have been suffering from a weird problem. Until last month, I used to masturbate once a day, but have stopped for three to four weeks. The condition I am noticing is that I can feel a couple of drops of semen stuck at the tip of my penis. When I rarely masturbate now, there is a healthy amount of semen discharge, but when I try to urinate, I can feel the urine being blocked at the tip of my penis. Thus, I have to apply extra pressure, which sometimes works and sometimes does not, in which case I have to wait for some time for the pressure to build up. It is cumbersome, and I am worried that it might have some more significant consequences down the line. The issue has been presenting for about seven to eight days, and for now, it is just an inconvenience, but I am afraid it might turn into something serious. I drink less water during the day; I wondered if that might have anything to do with it. Please help.

From the description, you have either inflammation at your meatus or have a stricture (stricture is a narrowing caused by fibrosis). Inflammation will resolve in a maximum of two weeks. But stricture will not. You can do a flexible cystoscopy to rule out stricture after two weeks from the first time you felt the obstruction. Stricture will require further investigations and treatment like uroflowmetry (to test the amount urine voided during urination) and bladder scan, retrograde and antegrade urethrogram, and flexible cystoscopy. All this can be done after the initial flexible cystoscopy to know the length of the stricture and decide on the treatment. I hope the above information was helpful.
