Patient's Query

Hello doctor,

I have PCOS. The doctor prescribed me Met and Normoz. After three months, he gave me Genotropin injection 75 UI from day 2 to 6 and Letroz from day 3 to 7. He said they are small-sized follicles, he abandoned the cycle and told me to go for ovarian drilling. I want a second opinion on that, please. Why do I not respond at all? I follow a diet and exercise.

Please help.

Hello,

Welcome to icliniq.com.

Your hormone profile suggests PCOD. What is the size of ovaries on ultrasound? If not done, you should do blood tests, sugar PP, TSH, prolactin, and testosterone. Often, oral contraceptive pills like Diane 35 (Cyproterone and Ethinyl estradiol) taken for three months help in increasing the size of follicles in the subsequent cycles. If your blood prolactin or testosterone level is high, then you will need medicine to normalize it.

Hope this helps.

Thanks and regards.

Patient's Query

Hello doctor,

Thank you for replying.

I have taken Metmorfin and Normoz for PCOD. I am sending you files. Can I try for pregnancy after taking Diane 35 for three months? Is there no way I can induce ovulation?

Please help.

Hello,

Welcome back to icliniq.com.

I have gone through the files (attachments removed to protect patient's identity).

You have not done serum testosterone. If it is high, you will need to normalize it by medicine. You can try for pregnancy after taking Diane, but not during the duration of taking it as it is a contraceptive.

Hope it helps.

Thanks and regards.

Patient's Query

Hello doctor,

Thank you for your reply.

What should I take for decreasing serum testosterone level? Do I need an ovarian drilling? Or will I be fine with Diane 35 for balancing my hormones?

Please suggest.

Hello,

Welcome back to icliniq.com.

You have not mentioned your blood testosterone level. Diane will antagonize high testosterone if you have it. Drilling is a form of treatment, but it does not normalize the hormones though it may help in ovulation. There is a chance factor in whatever treatment you do and no comments can be made with certainty.

Hope it helps.

Thanks and regards.

Patient's Query

Hello doctor,

Thank you so much.

I will send you the report. I will test tomorrow. Today, I had brown spotting. What could be the reason? Is it possible I am ovulating?

Please help.

Hello,

Welcome back to icliniq.com.

It is not period, but periovulatory spotting.

Hope it helps.

Thanks and regards.

Patient's Query

Hello doctor,

Thank you for your reply.

Does that mean I will ovulate and can have intercourse for pregnancy?

Please help.

Thanks and regards.

Hello,

Welcome back to icliniq.com.

Yes, there is always a chance of ovulation.

Thanks and regards.

Patient's Query

Hello doctor,

I went for an ultrasound today. My follicle on day-14 is 13.5, two dominant follicles seen. What should I do? I am also spotting. Will my egg mature?

Please suggest.

Hello,

Welcome back to icliniq.com.

You have to do a serial ultrasound to see if the follicles are growing, it may mature in three to four days.

Thanks and regards.

Patient's Query

Thank you doctor,

My dominant follicles are 18.1,19.5 mm on day-17. What should I do? Should I take the trigger shot?

Please help.

Hello,

Welcome back to icliniq.com.

You can wait for spontaneous rupture or can take trigger shot if your doctor has advised so. In any case, start having sex from today itself.

Hope it helps.

Thanks and regards.

Patient's Query

Thank you doctor,

Since I have PCOD, I thought maybe the follicle will not rupture. So, I thought of taking the trigger shot and have my eggs mature.

Please help.

Hello,

Welcome to icliniq.com.

Trigger shot can be taken but under the supervision of a doctor.

Hope it helps.

Thanks and regards.

Patient's Query

Hello doctor,

I am 7 weeks pregnant. Since I have PCOS, what complications will I have? I have been taking prosgesterone pills. Will I have any problem with the placenta after that?

Please help.

Hello,

Welcome back to icliniq.com.

You do not have increased chance of complications. I suggest you follow the instructions of your gynecologist.

Thanks and regards.

Hello doctor,

Thanks for your reply.

I have on and off spotting only for hours, twice every week for the past five weeks. Is it normal?

Please help.

Hello,

Welcome back to icliniq.com.

You may need to do ultrasound to check if all is well.

Thanks and regards.

Patient's Query

Hello doctor,

Thanks for replying.

I had ultrasound in seven weeks. I was diagnosed with subchronoic collection. Will I miscarry eventually. What are my chances? Is it dangerous?

Please help.

Hello,

Welcome back to icliniq.com.

The fetus is good. You may not require abortion. Take rest and take advice of your gynecologist regarding heparin injection. You must continue progesterone.

Hope it helps.

Thanks and regards.

Patient's Query

Hello doctor,

I am already on heparin injection. I do not want to abort.
