Hello doctor,

I have normal breakouts every once in a while. But last month I had severe acne all over my neck and left cheek. I started using a new brand of hair dye. My hair is longer than my shoulders. I am wondering if it could be due to the Henna-based hair dye. Even though it is natural. I have an IUD and no other medications. I follow a strict gluten-free diet due to celiac disease.

Thank you for providing information. It is inflammatory acne (adult-onset or hormonal). Henna-based hair dye or Celiac disease are less likely to be the cause in your case. If IUD is hormonal, then it can cause this kind of acne. Overall your breakout indicates hormonal disturbance. You can start Azelaic acid 20% cream in the morning for 20 days and Clindamycin 1% gel at night for 20 days and daytime non-oily or greasy sunscreen (Suncros Aqua Gel) and one face-wash Ahaglow (Aloe vera, Glycolic acid, and Vitamin E), tablet Prednisolone 5 mg one tablet at night after food and 2.5 mg one tablet at the morning after food for 20 days, tablet Azithromycin 500 mg one tablet at the morning after food (three days in a week) for four weeks. The above treatment looks lengthy, but due to detailed explanations and creams, face wash, sunscreen needs to be continued as a maintenance treatment. You can check for PCOS (Polycystic ovary syndrome) or serum hormonal levels. If you have any queries, you can ask. Thank you.

Can I find those creams and medication at any drugstore? I have also had my IUD for eight months now. Why would this acne be starting now?

Yes, you can get these creams and medications in drug stores; you can check availability over there. Your Acne are mostly due to hormonal disturbances; it could be due to monthly hormonal changes or can also be IUD (Intrauterine device)-induced hormonal changes that would develop over time or even any other reasons.
