Hi doctor,
My testicle is facing sideways all of a sudden. However, the front is facing the inner part of my thigh. I had a sonogram done, and no evidence of torsion was found, and my urologist did not seem too concerned about it. There is no pain, only a bit of discomfort when I walk because my thigh hits the testicle. Everything is working properly but it is just weird. I do not even remember my testicle hanging like that. Please let me know what you think.

The testis lies in the scrotum, and the scrotum can expand and shrinks due to temperature. It expands at high temperature, and testis will be hanging lower, and at low temperature, it will shrink, and testis will be higher. That is normal. The testicular torsion is a twist of the testis on its access. This leads to the compromise of blood supply to the corresponding testis to variable degrees depending on the twist's degree. Testicular torsion is a surgical emergency and should be corrected surgically. Torsion can be intermittent like there will be a twist associated with sudden intolerable pain. The pain will be resolved as testis untwist again. Testicular torsion is most commonly present at the age of 16 to 25 years. Since your ultrasound is normal, there nothing to worry about. But you just need to be careful as torsion can be intermittent. Therefore if you develop sudden pain, then you need to go to the hospital immediately. No medications should be indicated at this time for you.
