Hi doctor,

I had pimples on my penis a week ago. I had a doctor's examination but did not see it as anything serious, although I went to a sexual health clinic, and the doctor said there was a 75 % chance it was molluscum. Two other doctors told me that it may be folliculitis. These photographs uploaded were taken around five to six days ago. Let me know if you would like new pictures for examination.

I understand your concern. I reviewed your case history and the attachments (attachments removed to protect the patient's identity). My diagnosis would be folliculitis and Fordyce spots. Regardless, neither of them is sexually contagious. It does not look like a molluscum to me. A way to differentiate between folliculitis and Fordyce would be to look for similar spots on the scrotum, lips, and forehead.

I understand your concern.

If this is folliculitis or Fordyce spots, you do not need to do anything unless it is causing you pain or discomfort or if you want it removed for cosmetic purposes. For this to be molluscum, you should have a history of intimate contact with someone with it. Laser or Podophyllin (Salicylic acid) cream treatment may be a good option as it can potentially spread it. I suggest against a biopsy as it might leave unaesthetic scars. So, if there is no history of intimate contact with someone who has it, you do not need any treatment. I suggest you consult a dermatologist.

I appreciate your help.

I have just had my angiokeratoma bumps removed and a wart from the genital area from a dermatologist. They said not to go gym for around ten days. Can you confirm anything to avoid and how long it usually takes for the healing process? I also have questions, I would like to continue to go gym. Is it still fine that I go, or shall I still avoid it? Secondly, is there anything I should apply on it, such as coconut oil or petroleum jelly to heal the scars a lot sooner? The spots on the penis folliculitis have completely gone after three weeks after applying coconut oil, apple cider vinegar, and tea tree anti-fungal body wash. The reason I ask you the above questions is, because the procedure was pretty quick, and did not manage to get any questions in with the dermatologist, and they were really busy with other patients.

Thank you for your answer.

It was surgically removed through an electric laser. If possible, I will try to request pre-operative photographs to send you. In regards to the wart, I believe they lasered it and then cut it. But with the angiokeratoma, they lasered it. Also, in relation to the wart, it was around the Corpus spongiosum area of my penis that was brown and kind of peeling off the layer, and I have had it for, I presume, 24 months. I did not really know it was a wart until they told me. So, I requested they remove it. Would you recommend I get an HPV vaccine as 31 31-year-old old? And as the wart is removed, can they come back?

Sorry for the delay in getting back to you. If everything was lasered, I do not see why you cannot go to the gym. Just make sure you do not have tight underwear, as it may rub against the penis and cause irritation. Wear baggy clothing for a week or so until the scars heal. There is no need to apply anything, just a normal unscented moisturizer is fine. As far as warts are concerned, yes, they may or may not come back. There is no definitive way to tell. I would also suggest against intercourse for maybe two weeks to let the scars fully heal. I do not need to see the pre-operative photo. Just a current photograph will be enough for me to check out the scars.

Yes, as long as it is not perfumed. Just plain Nivea (lanolin alcohol, paraffin, and panthenol) is fine. I can see why they asked you not to go to the gym. There are multiple scab formations in the picture (the attachment is hidden to protect the patient's identity) due to the laser, and friction might cause them to fall off and cause irritation. But as long as you are wearing baggy underwear, you should be fine to go to the gym.

How long do you think it will take to fully heal? Can I apply coconut oil on the area, and can I use any body wash, or is anti-fungal body wash good? Do they usually leave scars after the healing process?

It takes a couple of weeks to fully heal. No need to apply anything other than a plain moisturizer. Generally, no, but then everyone’s body responds differently to healing, but on the whole, there will be minimal to no scars.

Generally, the vaccine is recommended between 9 to 26 years of age. However, if a person is at increased risk of HPV (human papillomavirus) contraction, they can get the vaccine, and three doses will be needed. Getting the HPV vaccine later in life will not cure HPV for someone who already has it, but may protect against other strains.

Looking at the images (attachments are hidden to protect the patient's identity), I do not think it is a wart. As far as HPV is concerned, we cannot say with confidence that someone does not have it. HPV exposure can happen even without a person developing warts. You would require an HPV, IGG (immunoglobulin G), and IGM (immunoglobulin M) test to determine active exposure or past exposure. Even if someone has had HPV exposure in the past, most people will eliminate the virus in two to three years.

Looks like you have scratched that pretty hard. I would advice you try a rash cream (baby rash creams will also work) on the area - twice a day for three to five days. Avoid soap on the area as that too may irritate it (just clean that area with water). Try keeping the area dry and sweating will aggravate the itch. Revert back to me if this does not settle down in a week.
