Hi doctor,

I am a 56 year-old-female with type 2 diabetes and hypertension. I am taking tablet Glycomet 500 SR, Coversyl 4 mg, Monti-FX, Alex SF. I have very severe dry cough throughout the day for the past two weeks. The doctor had a suspicion that Coversyl might cause cough. I was taking it for two weeks and had cough, skipped a day's dose due to some reason and the cough went away. Can Perindopril be the cause and what would be the alternative? I was taking Telmikind 40, before this.

I have read your concern and have gone through your reports (attachments removed to protect the patient's identity) in detail, and I will help you out.

1. Your doctor is partly correct. Cough is a very common side effect of Perindopril. It belongs to a group of drugs called ACE inhibitors, and these drugs are very well known to cause cough. The solution is to switch the medication. Instead of using ACE inhibitors to control bp, I suggest Amlong (Amlodipine) 10 mg or Carca (Carvedilol) 3.125 mg. These drugs are not associated with cough. Since you have a temperature of 99.2 degrees, I will suspect infection as the cause for cough. Hence, I would suggest a chest x-ray (to rule out pneumonia) and an antibiotic like Azithromycin 500 mg once a day for five days will be sufficient.

2. Blood sugar levels are well controlled, and HbA1c shows 6.5, which is good control. But I would encourage you to bring it a little further down to 6 with exercise and diet modification. This will prevent increasing the requirement of medicines for diabetes in the future.

3. Serum uric acid levels are high. If there are no symptoms, then no treatment is required at all. If there is joint pain (symptoms present), I would suggest tablet Febutaz (Febuxostat) 40 mg once a day to control it.

4. The reports show that there is a mild iron deficiency anemia, and since you mentioned that the patient's age is 56, I am pretty sure the patient has attained menopause. So bleeding cannot be a cause of this anemia. For this, I would suggest tablet Bandy (Albendazole) 400 mg once a day for three days (chew the tablet) to kill all parasites in the gut and tablet Orofer XT (iron supplement which is well tolerated) to recover from the iron deficiency.

Thanks for the reply. The doctor has switched Coversyl to another tablet, and after investigation, he prescribed additional medicines. I have done the X-ray of the chest (PA). I have been advised to hold on before doing HRCT or RTPCR. The current diagnosis is bronchitis. I am using a nebulizer with Foracort 0.5 mg respules in place of Budetrol 0.5 mg as I could not find the prescribed one. Feburic 40 was prescribed earlier.

I have seen all your reports. Everything seems fine. The treatment is also perfectly prescribed by your treating physician, and your uric acid levels are under control now as well. As I told you, Orofer XT (Ferrous ascorbate and Folic acid) is a good option to cover up your anemia. Your x-ray is also fine. You seem to be on the road to recovery.

I want to ask one question. Did you show my answer from here to your treating physician? Because he has treated you on similar lines as I would have done with my patient.
