How to manage my phimosis and hypospadias?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am a 25-year-old single male with an undiagnosed case of phimosis and hypospadias. Meatus was found between glans and shaft; foreskin works as a tube for passage and balloons. So urine flow and masturbation are normal when unretracted.

He can forcefully retract when flaccid but urine sprays when retracted. He cannot retract when erect, but ballooning is seen without spraying. When he tries to retract when flaccid and induced erection, after full erection, constricted below the glans is seen and painful. There is no previous surgery and no comorbidities and no current medication.

Please help.

Thank you.

Hi,

Welcome to icliniq.com

I can understand your concern.

Based on your description, you seem to have a combination of phimosis (foreskin too tight to retract) along with hypospadias (urethral opening on the underside of the penis rather than at the tip), and the tight foreskin is causing ballooning and difficulty in retraction, especially during erection. The pain and constriction you feel on erection after partial retraction suggest that the foreskin is tight and should not be forcefully pulled, as this can lead to injury or worsening tightness.

Since you are able to pass urine and masturbate without major issues when the foreskin is not retracted, it indicates functional adaptation, but the symptoms you describe (ballooning, painful constriction, spraying on retraction) mean this condition does need proper management.

In mild cases, gentle retraction exercises and short-term use of topical steroid creams can help, but in your case, given the associated hypospadias and functional issues, this may not be sufficient alone.

The important advice is to avoid forceful retraction, maintain good hygiene externally, and monitor for any pain, infection, or difficulty in urination. Definitive correction, if required, is usually surgical and depends on severity, but many cases are manageable once properly evaluated.

You can share more details or images if comfortable, and I can guide you further step by step in the best possible way.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
