Hi doctor,

I am a 25-year-old male, 5 feet 10 inches. I am not sure if I have skin pigmentation, melasma, or melanoma. It showed up three years ago as a little mark, and I did not think anything about it then. But one day, when I woke up, it looked like a bruise or pigmentation all around it. I went to the doctor and got a blood test to check my thyroid levels, but it was normal, and we could not figure out what it was. So I am unsure completely. I used a steroid cream and it helped it fade a little, but I am still worried. I do not have a dermatologist near my place, but I need to travel three hours to get an appointment. And my doctor has no idea what it could be, so I am unsure and figured I would take steps to get another opinion.

Hi doctor,

Thank you for the reply.

It resembles a triangular shape other than the mark in the middle. I do not know if it is me overthinking it, but it does itch around every once in a while, but not frequently in any way. I have had a sniffly nose for a year or so, but I do not think it is related. I have no allergies that I know of, insect bites, or hives. It is not recurrent and has not gotten bigger. It almost looks like it spread up the tiniest bit, but I am a little unsure, and it is not inflamed. My throat has not been sore; looking back at the photos, I have seen it was just a little mark.

It looks like steroid-induced hyperpigmentation (attachment removed to protect the patient's identity). I suggest you to follow the instructions mentioned below,

1. Apply 10 percent Aziderm cream (Azelaic acid) during the daytime for 20 days.

2. Apply Kojic cream (Kojic acid, Lactokine Fluid, and Axeloglicina) at night for 20 days.

3. Use Kojic soap (Kojic acid, Arbutin, vitamin E) for a bath for one month.

4. Avoid scratching and rubbing of lesions.

5. Take the tablet Teczine-M (Levocetirizine and Montelukast) once at night after food for seven days (if there is itching).

6. Moisturize skin daily with Venusia max cream (Aloe butter and Shea butter) immediately after bath for one month. Moisturize your skin frequently.

7. Drink seven to eight glasses of water daily.

8. Use a photostable sunscreen 20 minutes before going outdoors. Avoid exposure to direct sunlight.

9. Stop using the steroid cream.

10. Take the tablet Limcee (Ascorbic acid, Sodium ascorbate, and Zinc citrate) 500 mg once a day after breakfast for 30 days.

11. Take the tablet Antoxid HC (nutritional supplement) once daily after dinner for 30 days.

12. Avoid exposure to dust, pollutants, pollens, and animal dander.

13. Avoid known triggering factors.

Consult a specialist, talk with them and take the medications with their consent.

1. Is it inflamed at present?

2. Is there any redness?

Hi doctor,

Thank you for your response.

I did not take any steroid cream until I noticed the mark was getting bigger and got a thyroid test. Then after that, it looked fine. I tried the cream to lighten it, this is what it looks like right now, but above the mark, it looks like it did spread a little. I am seeing my doctor again, so I will ask them for advice and where to get all of those medications. I will no longer use that topical steroid cream, and I have not used it for a couple of months.

I suggest you to apply low-potency steroids like Hydrocortisone cream or Desonide cream at night for ten days. I suggest you to take the tablet Omnacortil (Prednisolone) 10 mg once a day after food for three days (if there is swelling). Kindly consult a specialist, talk with them and take the medication with their consent.
