Patient's Query
Hello doctor,
I have been experiencing skin issues in my underarms for the past two to three years. Initially, I used to get frequent pimples in that area. Although they have mostly healed now, the underarm skin still does not look or feel normal. The natural lines in the underarms appear very deep and folded, almost like holes. The texture of the skin seems irregular, and there are visible dark spots where the pimples once were.
Even though the pimples are gone, it still feels like something is inside the skin, and it hurts when pressed or pinched. Occasionally, there is discomfort or pain in those areas. I have been using Tretinoin regularly and applying it correctly, but I have not seen much improvement in the appearance or texture of the skin. I am concerned about these persistent folds, discoloration, and the occasional pain, and would like guidance on what this might be and how to treat it effectively.
Kindly suggest.
Hello,
Welcome to icliniq.com.
Thank you for sharing your concern. I am genuinely sorry that you are going through this. I can imagine how physically and emotionally uncomfortable and frustrating it must feel.
From what you have described, it appears to be a combination of old acne scars, possible trapped inflammation beneath the skin, and hyperpigmentation that has developed over time. The deep folds and hole-like indentations you are noticing in your underarms may be the result of repeated infections, blocked hair follicles, or a condition known as hidradenitis suppurativa. This is a chronic skin condition where painful bumps and abscesses form under the skin, particularly in areas such as the underarms.
Although the pimples may have healed superficially, the lingering pain and the sensation that something is still "inside" could indicate that there is still some underlying inflammation or unresolved scarring. The dark patches and pronounced lines you are seeing are most likely due to post-inflammatory pigmentation and skin thickening. This is especially common when the skin has undergone repeated cycles of swelling, discomfort, and healing. In some cases, frequent scratching, pinching, or the use of harsh deodorants may contribute to further irritation, sensitivity, or worsening of the skin’s appearance.
You deserve proper relief and healing, and it is important to know that you do not have to live with this quietly or alone. I strongly encourage you to consult a dermatologist, who can provide an accurate diagnosis and a personalized treatment plan. They may recommend options such as gentle exfoliating agents, topical Retinoids, or even laser therapies to reduce the appearance of the folds, scars, and pigmentation. If this turns out to be hidradenitis suppurativa or a similar condition, there are medical therapies available that can help manage flare-ups and prevent further scarring.
In the meantime, it would be helpful to use fragrance-free, non-comedogenic products, keep the underarm area clean and dry, and wear loose, breathable clothing to minimize friction and irritation.
I hope this information has been helpful.
Regards.
