What could cause anal itching after passing a stool?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am going to share three problems with you. Kindly guide me towards the correct solution.

I am having dark yellow-colored urine, but most likely it is due to a severe lack of water. I am not drinking anything. The frequency has dropped from four times a day to two times a day.

I have been sleeping excessively for three days. I have been facing this problem on and off for three months now. At first, it seemed to go away, but for the past week, I have been feeling it again.

I feel like there is a worm in the excrement. I tried to look for worms but could not find any. However, I do feel itching, as if there is a worm in the lower region of the large intestine, near the anus. The feeling always comes after defecation.

If you would be so kind, can you find or direct me towards something that would help me?

Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

For your dark yellow urine, the likely cause can be dehydration. You are passing urine only twice a day, which shows very low water intake.

I suggest you follow the instructions mentioned below:

  1. Drink half to one gallon of water daily (spread throughout the day).

  2. Avoid too much tea or coffee, as they dehydrate more.

  3. If urine stays dark despite adequate fluid intake, do a urine routine test and liver function test (LFT) to rule out any liver issues.

For excessive sleep (past three days), possible causes are as follows:

  1. Fatigue or viral infection recovery.

  2. Thyroid imbalance (hypothyroidism).

  3. Anemia or low blood sugar.

I suggest you undergo the following tests:

  1. Complete blood count (CBC).

  2. Thyroid-stimulating hormone (TSH).

  3. Fasting blood sugar (FBS).

I suggest you check for any medications or recent stress. Try maintaining fixed sleep timings and hydration, as low energy can worsen from dehydration too.

For itching and a worm-like feeling near the anus, the likely cause is pinworm (Enterobius vermicularis) infection. Commonly causes itching around the anus, especially after stool or at night. You may not always see worms in stool.

I suggest you follow these steps:

  1. Take a tablet of Albendazole 400 milligrams as a single dose at night after food. Repeat after two weeks.

  2. Treat all family members simultaneously.

  3. Maintain strict hygiene.

  4. Wash your hands before eating and after using the toilet.

  5. Change undergarments daily and wash bedding in hot water.

  6. Keep nails short and clean.

I hope that this answers your query.

Kindly follow up if you have more doubts.

Thank you.
