Hi doctor,

I am 41 years old and have a 10-year-old son. I am trying to conceive again. I am using digital ovulation test for the first time. I have two peak fertility as per the test. Clearblue and first response pregnancy test is negative within 10 minutes specified time but shows a faint line after one hour or so. My period is due after four days. Why am I having two peak fertility this cycle? When will the ovulation take place? Are there any chances I might already be pregnant and the clear blue ultra early pregnancy test shows a false negative?

I understand your concern.

If you have experienced two fertility peaks through an advanced digital ovulation test kit. In that case, it is surprising that twin LH (luteinizing hormone) surges are not experienced in a woman unless she has been exposed to ovulation induction drugs for many months. In that case, each ovary may ovulate irrespective of the other and exhibit twin LH peaks. But this is rare. You may get a blood serum beta hCG (human chorionic gonadotrophin) test after 14 days post intrauterine insemination to check for successful conception. If the hCG levels are more than 5 mIU, then you have successfully conceived, else you have not.

Why is there a second LH surge with peak fertility? Is it possible that ovulation happens four days before the period? Are there any chances of pregnancy from my details even if the Clearblue ultra early pregnancy test results are negative, which can be used six days before the period?

I understand your concern.

The second LH (luteinizing hormone) surge is difficult to explain. The next test would be to get a quantitative blood test for LH serum levels which can tell us the present levels of LH. At 40 years of age, this could be perimenopausal as well. How many days of cycle do you have? Also, chances through self-IUI (intrauterine insemination) and timed use of an ovulation kit remain 35 % to 38 % during each cycle universally.

Thanks for the reply.

My usual cycle length is 28 days, and my next period is due after four days of the second positive ovulation peak fertility days. I did an ovulation test again today, and it was negative but positive a few days back. My pregnancy test is also negative today. But some of my pregnancy tests during the last few days showed a faint line after a few hours. I did a qualitative blood test today. The results will come late. What are the reasons for positive ovulation only four days before the due date of my period?

As I mentioned, there might be delayed ovulation from the second ovary, but the possibility is rare. Clearblue test, as it claims, may be detecting a false positive result. Confirmation can be made by getting serial serum LH (luteinizing hormone) levels now in your next cycle if you do not become pregnant this cycle. Let us wait for the qualitative blood test, as your cycles have been regular, so we would take the first LH surge as ovulatory in your case and wait for the results.
