Patient's Query

Hello doctor,

I need a review of my review of my ECG ( electrocardiogram), which I am attaching. I have premature ventricular contractions (PVC), and I have anxiety. I want to go to the emergency room and do ECG. Can you look at it, please? Can you see the PVCs? The doctor said the ST was abnormal. I did a blood test negative for troponin. I did an echocardiogram which was normal limits, and a stress test. The PVC (Premature ventricular contractions ) has gone away for one year now. I feel I have as many as eight per day, sometimes in a row. Is it dangerous? Do I need treatment? I read online they can cause cardiomyopathy or death. I have another question my heartbeat at rest is 58 per minute. Can I take Clonazepam for anxiety at such a low rate? The doctor also said I might have repolarisation. I am worried. Please help.

Hi,

Welcome to icliniq.com.

Do not worry. In this ECG strip, I do not find any PVC. Your ECG is within normal. Your heartbeat is 58/min at rest. It is fine. You need not take any medicine for that. For your anxiety, I suggest you follow non-pharmacological treatment. Do breathing exercises for five minutes two times a day. Do meditation for five minutes daily. Do walking for 30 to 45 minutes daily will help you. I hope this helps. Regards.

Patient's Query

Hello doctor,

Thank you for the response. I do have premature ventricular contractions every day. Sometimes I feel lightheaded and find it hard to breathe.Last year's echocardiogram was normal and ejection fraction was 76, a little high, and the carotid TSA DOPPLER was normal. Should I do a calcium score or another test? Sometimes I have six or seven premature ventricular contractions daily, sometimes one or two only. Why is it like this? Please advise.

Hi,

Welcome back to icliniq.com.

You can do that based on your age and associated anxiety if you want to do a calcium score. Regarding Holter monitoring, as suggested is not impractical. I hope this answers your query. Regards.

Patient's Query

Hello doctor,

Thanks for the response, and I have a few more doubts. If my Doppler is good,do I possibly have some reduced flow in the heart from atherosclerosis? Obligated Holter, or can I choose to stop now and not worry? Can I exercise, run and work out, even with PVC? Do you think I could have ischemia or heart disease? Please clarify, doctor.

Hello,

Welcome back to icliniq.com.

To exclude ischemia, the gold standard is the coronary angiography test. In case of doubt, one can go for this. I hope I have cleared all your doubts. Regards.

Patient's Query

Hello doctor,

Does that mean that I could have ischemia? Thank you.

Hello,

Glad to have you back!

I am here to help you get the best medical advice. You should walk for 30 minutes daily at a normal pace. During the walk, you check PVCs (premature ventricular contractions) if possible. Avoid strenuous physical activities. I hope your query got resolved. Please feel free to reach me again, in case of further queries. Thank you.
