Patient's Query
Hi doctor,
What is the recommended dose of Propofol during ORIF surgery on a fractured wrist? The patient is a 57 year old female. Her weight may be about 140 pounds. The surgery may take 30 minutes or so.
Hi,
Welcome to icliniq.com. Certainly, the dosage of anesthesia varies based on individual patients' health conditions, including factors like age, lung health, and kidney function. For patients aged 55 and above, the induction of anesthesia begins at 1 to 1.5 mg/kg of body weight. During the maintenance phase, the dosage is adjusted between 3 to 6 mg/kg/hr, considering factors like the patient's musculoskeletal response, surgery duration, and overall condition. In cases of monitored anesthesia care sedation (MAC), most patients require an IV infusion of 25 to 75 mcg/kg/min (1.5 to 4.5 mg/kg/hr) or bolus doses of 10 mg or 20 mg, adaptable according to real-time surgical circumstances.
Patient's Query
Thank you doctor,
The surgery is MAC and a physician's assistant will be in or monitoring the patient. Should we go a little high or a little low? Is it unlikely to be able to change dose once started?
Hi,
Welcome back to icliniq.com. I value the health query you've presented.
It might be advisable to opt for a slightly lower dosage. If necessary, an incremental bolus dose could be considered.
Additionally, it's crucial to take into account the patient's other health factors during the decision-making process.
