Hello doctor,

My biopsy results are serious, and I cannot stop panicking. At age 50, can a Gleason score of 9 still be survivable? I feel physically well, which makes this even harder to process.

  1. Does a score that high always mean the cancer has spread or is at a late stage?
  2. How much does being younger help with survival and treatment success?

I am scared that aggressive treatment could significantly affect my quality of life, but I am also terrified of doing too little.

A Gleason 9 prostate cancer is a serious, high-risk cancer, but it is not automatically fatal or incurable. The Gleason score tells doctors how aggressive the cancer cells appear under the microscope; it does not determine whether the cancer has spread.

Some men with Gleason 9 disease still have cancer confined to the prostate and can receive treatment with curative intent.

Your age of 50 years is generally a positive factor because younger patients often tolerate treatments such as surgery, radiation therapy, and hormone therapy better and may achieve good long-term outcomes.

Survival depends much more on the stage of the cancer, PSA (prostate-specific antigen) level, imaging results, and response to treatment than on the Gleason score alone.

Although treatment may influence quality of life, especially in relation to urine and sexual functions, these should be weighed against the advantages of treating a highly aggressive type of cancer. The next crucial step is determining whether the cancer is localized or has spread.
