How to manage my elderly father's advanced prostate cancer?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My 68-year-old father was recently informed that his prostate cancer may have spread to other areas, possibly his bones or liver. We're currently waiting on scan results to confirm this, but I'm trying to understand what this could mean for his overall prognosis.

If the cancer has metastasized, how does that affect treatment options? Are there still effective ways to slow down or manage the disease at this stage? He's otherwise been in good health, so we're holding on to hope while also preparing ourselves for what's ahead.

As we get ready for the next appointment, what important questions should we ask the oncologist to better understand his condition and the road ahead? Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and understand your concern.

If your dad’s prostate cancer (a type of cancer that begins in the prostate gland, which produces seminal fluid) has possibly spread to other areas like the bones or liver, that can understandably be scary to hear. While this does make the situation more complex, it doesn’t mean it’s untreatable, or that you should lose hope. Once prostate cancer becomes metastatic, the goal of treatment shifts from curing it to managing it: slowing the cancer’s growth, controlling symptoms, and helping him maintain the best possible quality of life.

Common treatment options include:

  • Hormone therapy – This helps reduce testosterone, which prostate cancer cells often need to grow.
  • Chemotherapy – Used to kill fast-growing cancer cells throughout the body.
  • Radiation therapy – Often targeted to areas like the bones to ease pain or other symptoms.
  • Targeted therapy or immunotherapy – These may be options if genetic testing shows specific mutations.

At your next oncology visit, consider asking:

  • Where exactly has the cancer spread?
  • What’s the main goal of treatment right now: slowing it down, managing symptoms, or both?
  • What side effects can we expect, and how are they handled?
  • How will treatment affect his day-to-day life?
  • Are there any clinical trials he may qualify for?
  • What support is available for pain management, diet, and emotional well-being?

The fact that your dad has been in good overall health is definitely in his favor. It can help him respond better to treatment. This is a challenging time, but staying informed and asking the right questions will help you feel more in control as you move forward together.

I hope this helps.

Please revert so I can assist you further.

Thank you.
