Hello doctor,

I have been struggling with prurigo nodularis for nearly two years, and it is becoming unbearable. I have hard, intensely itchy nodules all over my arms, legs, and back, now starting to appear on my chest and neck as well. The itching feels like fire ants crawling on my skin and is relentless. The nodules often start as small bumps but grow larger when I scratch, which I cannot stop, especially at night.

Despite trying numerous topical treatments and phototherapy, nothing provides lasting relief. I even ended up in the ER last month with a skin infection from excessive scratching. They prescribed antibiotics and told me to follow up with a dermatologist, but my appointment is still six weeks away.

This condition has completely disrupted my life:

  1. Sleep is nearly impossible; I often wake up to bloody sheets from scratching.

  2. I recently lost my job as a server, as customers were uncomfortable seeing my exposed arms.

  3. My bloodwork showed elevated eosinophils (720), which my doctor said could indicate ongoing inflammation or allergic response.

  4. Gabapentin was prescribed for the itching; it helps a bit, but leaves me too drowsy to function during the day.

I am desperate at this point. Are there any new or advanced treatments available for prurigo nodularis, possibly even experimental or biologic options, that could provide meaningful relief?
