How to manage stiff joints in psoriatic arthritis?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 40-year-old man, and my doctor told me I have psoriatic arthritis, which has made my fingers and knees stiff and sore almost all the time. It is hard to do normal things like typing or even lifting heavy stuff, and it feels like the pain does not go away. My doctor started me on some medicine, but I feel it makes me even more tired than usual.

Will this medication be enough to keep the arthritis from getting worse, or is there more I need to do? Are there specific exercises that can help with the stiffness and pain without making it worse?

I am also curious if there is a chance for it to go away eventually or if I will have to manage it forever.

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and understand your concern.

Medications you can include are-

1. Non-steroidal anti-inflammatory drugs (NSAIDs): These help reduce pain and inflammation but do not slow the disease down.

2. Disease-modifying anti-rheumatic drugs (DMARDs): Help to control the disease and prevent further damage to your joints.

3. Biologics: If DMARDs are not effective, biologic medications can be used. These target specific parts of the immune system responsible for inflammation. They might take a few weeks to show improvement but can be very effective in managing PsA.

4. PsA is generally considered a lifelong condition. However, early treatment and consistent management can reduce symptoms, prevent flare-ups, and help avoid irreversible joint damage.

Exercise that does not put too much stress on your joints like:

  1. Low-impact aerobic exercise.
  2. Swimming or water aerobics.
  3. Cycling.
  4. Walking.
  5. Stretching and flexibility.
  6. Yoga.
  7. Simple hand stretches, gripping exercises, and finger bending can help keep your hands mobile.

An anti-inflammatory diet can help manage symptoms. A balanced diet with plenty of fruits, vegetables, and whole grains. Avoid processed foods, sugary snacks, and excessive alcohol, as they can trigger inflammation. Stress can exacerbate PsA symptoms. Meditation, deep breathing, and relaxation techniques can help reduce flare-ups and improve overall well-being. Regular appointments with your rheumatologist are necessary.

I hope this helps.

Thank you.
