How can I manage my premature ejaculation problem?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have a premature ejaculation problem. When I try to start intercourse, I ejaculate immediately within 30 seconds. I am very nervous about this.

Kindly advise.

Hello,

Welcome to icliniq.com.

I understand your concern.

There are several potential causes of premature ejaculation. These include stressful work conditions such as night shifts or around-the-clock jobs, as well as underlying medical conditions like diabetes, hypertension, and thyroid disorders.

Managing your blood sugar levels, blood pressure, and thyroid hormone levels can contribute to improvement. Long-term use of medications for high blood pressure or psychiatric conditions may also have an impact.

Additionally, frequent or prone masturbation, excessive pornography use, insufficient foreplay from a partner, or a lack of understanding about foreplay and female sexual response can play a role. Psychological factors such as guilt from an extramarital affair, lack of physical activity, and obesity may also contribute.

It is advisable to check your fasting blood sugar (FBS) and postprandial blood sugar (PPBS) to rule out diabetes. Monitoring your blood pressure and conducting a thyroid panel, free T3, free T4, and thyroid-stimulating hormone (TSH) is recommended to assess thyroid function.

As a temporary measure, you may consider using Dapoxetine 30 milligrams (brand name: Duralast), which is commonly prescribed for premature ejaculation. However, it is important to consult a healthcare provider before starting any medication.

Natural methods can also be effective. Increasing the duration of foreplay can enhance sexual satisfaction and help with control. Ideally, women should receive 15 to 20 minutes of foreplay, while men should receive 10 to 15 minutes to ensure proper blood flow to the penis, resulting in a stronger and longer-lasting erection.

Foreplay should be performed in a relaxed and attentive manner. Your partner should understand how to provide stimulation in a way that helps you enjoy the experience without causing early ejaculation.

Incorporating relaxing music, using delay condoms, and focusing on emotional intimacy can also be beneficial. Remember, sexual intercourse is not just about penetration or duration. True satisfaction often comes from emotional connection and mutual understanding. Take the time to love your partner, learn about female sexual needs, including masturbation, and aim to provide a fulfilling experience that includes orgasm for your partner.

I hope you are satisfied with my answer.
