Hello doctor,

I'm having an unbearable itch in my private area, and this is lasting for almost a month. I didn't think much about it at first, but since nothing much seemed to improve, I wanted your advice.

I also have an itchy fishy odor. Based on my Gram stain result, I have high Gram-negative. I have searched online, and it is BV.

I haven't been taking anything yet, and I really need some help in this regard. I need your help in knowing what could be going wrong and what sort of tests I might need.

Moreover, please let me know about the medicines that I should take for this condition.

I have gone through your question and understood your problem.

I am really worried about your problems. Given your complaints of excessive itching externally, foul smell and presence of gram negative bacteria as shown in the Gram stain test (attachment has been removed due to patient confidentiality) I feel that you might be suffering from bacterial vaginosis.

Bacterial vaginosis occurs when there is an imbalance between good and bad bacteria, thus resulting to excess bad bacteria. The usual symptoms are:

Bad smell, specially during and after sexual intercourse.

Thin gray or white discharge.

Mild irritation or itching.

However, at the same time severe external itching can also sometimes mean there is a yeast infection. Bacterial vaginosis treatment usually includes:

Metronidazole tablets for seven days.

Vaginal Metronidazole gel.

Clindamycin cream.

These drugs would normally need to be prescribed, so you would have to get an appointment with a physician or clinic.

Also, it is important not to self-treat yourself randomly because the symptoms can get worse with wrong medication.

Please follow the instructions mentioned below.

Avoid douching.

Avoid scented soaps or washes.

Prefer using mild, and unscented soap externally.

Wear loose cotton underwear.

Avoid sexual intercourse or use protection until treated.

If you develop pelvic pain, fever, or worsening discharge, seek care urgently. I know it is frustrating sitting with results for a month. You are not alone in that.
