Hello doctor,

I am a 49-year-old female who was diagnosed with rheumatoid arthritis (RA) three years ago. I have been on Methotrexate (15 mg weekly) along with folic acid supplementation and occasional Prednisone (5 mg during flare-ups). Despite this, I have been experiencing more frequent flare-ups over the past few months, with persistent pain and swelling in my hands, wrists, and ankles, especially in the mornings.

My recent blood work showed an elevated erythrocyte sedimentation rate (ESR) of 56 mm/hr and C-reactive protein (CRP) of 9.2 mg/L, along with a positive rheumatoid factor of 48 IU/mL and anti-cyclic citrullinated peptide (anti-CCP) antibodies. An ultrasound of my hands revealed synovial thickening and early signs of joint erosion.

I am also struggling with fatigue, and my quality of life has been significantly affected. I am concerned that my current treatment may not be adequately controlling the inflammation, and I would like to know if it is time to consider adding a biologic or Janus kinase (JAK) inhibitor to prevent further joint damage.

What would you recommend as the next step to better manage my condition?
