Hello doctor,
I am very scared. I was bitten by a bat six months back but could not get the rabies vaccine. Since I was previously vaccinated, I received two booster shots. However, about six days before the shots, I started experiencing throat tightness and difficulty swallowing. Two months later, I developed double vision, which has been worsening, along with random body twitching. My swallowing has slightly improved but remains inconsistent. I am extremely concerned about these symptoms. What could be causing them?

Thank you for the reply.
I was vaccinated for rabies about one year and eight months ago after a raccoon attacked me ( I was living in a very rural area at the time). The bite sight from the bat was on my neck. I did not develop any anaphylactic reaction previously from the rabies vaccine. Before I received the booster rabies vaccine I had some extra salivation from my mouth but I was not drooling. This symptom has gone back and forth but seems to be getting a little better day by day. I am noticing my eyes tearing up daily still today, this is not from crying. I do not notice any periorbital pain in my eyes. I had some fear about drinking liquids (right before I received the first booster rabies vaccine) especially because of my throat tightness. Luckily my throat tightness and ability to swallow have been getting better slowly over time. I am drinking juice right now. I do still have times when I have some difficulty swallowing. Thank you so much for your help. I appreciate everything.

Anaphylactic reactions can cause chest tightness and difficulty swallowing, but other conditions like acid reflux, diffuse esophageal spasms, or anxiety neurosis may also contribute to these symptoms.
To determine the cause of your symptoms, tests such as saliva, blood, or cerebrospinal fluid PCR (polymerase chain reaction) for the rabies virus are necessary. Additionally, an MRI (magnetic resonance imaging) of the brain and an ophthalmologic evaluation can help identify possible muscle weakness. It is crucial to rule out rabies first before considering other diagnoses. Checking serum potassium, vitamin D, and hemoglobin levels is also recommended. Consulting a neurologist for further evaluation and reviewing test results is advisable.
