Does a small fissure cut on the glans penis need treatment?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have a small superficial fissure or cut on the glans penis that started with redness about ten days ago, likely due to friction or dryness. There was one episode of slight bleeding from the skin during an erection. The area has now closed but appears pink or red and sometimes looks circular.

A few nearby red spots appeared earlier but have since resolved. I experience mild irritation and occasional mild pain, but there is no severe pain, pus, or discharge. I do not have a fever, burning sensations during urination, and I have not engaged in any sexual activity. The redness sometimes increases temporarily after an erection. I am keeping the area clean and dry and avoiding friction. Please advise if this is normal healing or if any topical treatment is required.

Please guide.

Hello,

Welcome to icliniq.com.

I have read your query and understand your concerns.

Based on your description, this appears most consistent with a small superficial fissure of the glans skin, likely caused by friction, dryness, or temporary tightening of the foreskin (mild phimosis).

Sometimes a minor local infection or irritation can make the foreskin tighter, which increases friction during erection and can lead to small cuts. The fact that the area bled slightly once, has now closed, and shows pink or red skin with only mild irritation and no pus, discharge, fever, or urinary symptoms is generally reassuring and suggests that it is in the healing phase.

During healing, it is common for the skin to appear pink or reddish and slightly sensitive, and the redness may temporarily increase with erections because the skin stretches and blood flow increases. Since the nearby spots have already resolved and the wound has closed, this further supports normal healing rather than an active infection.

For now, you can continue gentle local care: keep the area clean with plain water, dry it well, avoid soaps or harsh chemicals on the glans, and avoid friction or excessive handling.

Wearing loose cotton underwear can also help reduce irritation. If needed, a mild healing ointment, such as plain petroleum jelly, or a simple antibiotic cream (like Mupirocin), applied in a very thin layer twice daily for five to seven days, may help protect the skin and support healing.

Most minor fissures usually heal completely within one to two weeks. If not, one needs to rule out balanitis, fungal infection, or significant phimosis.

At present, based on your symptoms, this sounds like a minor friction-related fissure that is healing, and with proper care, it should settle gradually.

I hope this clarifies the situation. If you have any questions or need further assistance, please do not hesitate to ask.

Thank you.
