Hello doctor,

I have a rash on my groin and vulva with severe burning, redness, and itching. Nothing is helping me. The burning and itching are next level, and I cannot sleep. I cannot even apply water, and even cloth causes burning.

A little friction from hair also seems to cut the skin. I have had this for one month. I have applied Clotrimazole cream (CLOCIP) and Zinc oxide cream (Himalaya diaper rash cream).

Please help me.

I can totally understand what you are going through.

The burning and itching in your vulval and groin area can be really bad especially when it keeps you awake at night and makes it hard to do things during the day.

It seems like you have intertrigo which is also causing some problems like contact dermatitis and a fungal infection. So we need to lower inflammation and get the infection under control.

Intertrigo is a condition that happens when the skin in areas like the groin, vulva and inner thighs gets inflamed. This usually occurs when it is hot and moist and there is not air getting to the skin. When this happens the skin can get weak. Start to feel sore and itchy.

When the skin is damaged it can get infected easily with a fungal infection like Candida. In the groin and vulval area this can cause a lot of pain and discomfort when walking or washing. Even wearing clothes can be painful.

The problem is, when the skin is already inflamed, using a lot of creams or washing it much can make it worse.

There are some things that can make intertrigo more likely to happen such as:

* Excess sweating

* humid weather

* Wearing synthetic clothes

* Being overweight

* Sitting for a time

* Friction on the skin

* Wearing sanitary pads for long

* Having diabetes

* Using too many skin products

To treat intertrigo we need to reduce the inflammation and help the skin heal. Once the skin is calm we can treat any infections that're present.

This is why we often use steroid creams for a short time along with antifungal medication if we think there is a fungal infection.

With the treatment you can get quick relief from the burning and itching.. It is also important to prevent it from happening again. Keeping the skin clean and dry wearing clothes and not using too many products can help.

Taking care of your skin can help it heal and stop the intertrigo from coming back. This is a condition that we can treat. The skin is still able to heal.

To treat it we can use a cream that has Clotrimazole and Hydrocortisone in it. You can apply a layer to the affected area twice a day for seven days then once a day for another seven days.. Do not use it for more than 14 days.

The Hydrocortisone in the cream helps to calm down the inflammation while the Clotrimazole helps to control the infection. Using both together can help to relieve the symptoms and let the skin heal.

To help with the itching and burning you can take a tablet of Cetirizine 10 mg twice a day, for seven to 10 days. This can help to reduce the itching and burning and improve your sleep.

I think you should talk to your doctor before taking any medication and make sure they agree with the treatment.
