I have recurrent red rashes that resemble a fungal infection, with moderate itching. They appear as red spots.

I understand your concern.

Based on the picture you have shared (the attachments were removed to protect the patient's identity), the appearance of the lesion and the history of a recurrent red, itchy rash that temporarily improves and then returns are most consistent with tinea incognito.

This is a type of fungal infection that has been altered and worsened by the use of steroid creams. Steroid creams reduce redness and itching for a short time, giving a false sense of relief, but they suppress the local immunity of the skin. As a result, the fungus continues to grow underneath, causing the infection to recur, spread, and appear atypical as red spots or patches rather than the classic ring-shaped fungal rash. Being overweight, excessive sweating, and friction can further increase the risk and recurrence of such fungal infections.

The rash requires proper antifungal treatment. Since you have used steroid cream previously, for the first seven days, apply the combination cream Hydrozole (steroid and antifungal) in a thin layer once daily in the morning on the affected area, and apply Bispor cream (Bifonazole 1%) once daily at bedtime. This regimen is intended only for short-term control of redness and itching.

After seven days, stop Hydrozole completely and continue Bispor cream (Bifonazole 1%) at bedtime for three weeks. Very importantly, continue the antifungal cream for an additional two weeks even after the rash and itching have completely cleared, to prevent recurrence of the infection.

Avoid direct sunlight and do not share towels or clothing with others, as the condition is contagious.

With this treatment regimen, the infection should resolve completely.
